Telemedicine

Alpine Family Practice now offers telemedicine visits. Often telemedicine can be used for the management of chronic diseases as well as the management of acute patient needs. Telemedicine is ideally suited for use when a physical exam is not needed. Examples include:

Hypertension

Diabetes

Depression

Chronic medication adjustment

Weight Loss

Counseling

Lab and imaging result follow up

Telemedicine visits are covered the same as an office visit with your insurance.

When you call to make an appointment please tell the front office that you would like to be seen for a telemedicine visit and click on the link below to enter the virtual waiting room at the time of your appointment.

In the state of Colorado we must have a consent form for telemedicine. Click below to download form and after filling it out you can email it to AnaP@myupdox.com so we can put it in your chart. You can also call the front desk to have this sent to you. If you filled this out in our office you do not need to do it again. There is also a link to an instruction sheet below.
